Welcome to SGPN
Welcome to South Georgia Physician Network, LLC. The organization is an independent physician association which is dedicated to advancing the practice of quality medicine in a managed care environment. Its members are primarily located in Valdosta, Georgia with physicians also located in Adel, Nashville, Tifton, Homerville, Lakeland, Hahira and Quitman. The physician practices are affiliated with South Georgia Medical Center, Smith Northview Hospital, Tift Regional Medical Center, Louis Smith Memorial Hospital, Berrien County Hospital, Clinch Memorial Hospital and Memorial Hospital of Adel. The organization currently has approximately 165 participating physicians and is managed by a Board of Managers which consists of nine physician members.
Mission Statement
Our vision is to bring unity to the market, provide insurers and self-insured employers access to a physician provider network, and to enhance the quality of care in our community. We continue to look for ways to bring the citizens of South Georgia quality health care at affordable rates.
Patients
Patients can locate SGPN primary care physicians and specialists at Physicians. By accessing Health Plans, the patient can determine if an SGPN physician participates in their health plan.
Physicians
Physician applicants can complete the application process at Membership. Once a member, physicians can refer patients to specialists by identifying Physician providers that participate in Health Plans available through SGPN. They can also obtain updates posted under News and identify Services offered by SGPN.
